New, from the factory, they come with spares. Used, they may or may not have a spare.

A lot of newer cars DO NOT have a spare tire, OR they come with a compact "donut" spare instead of a full-size one. It's supposed to save weight and lighter weight means better fuel economy. MOST new pickup trucks DO come with a spare wheel and tire, but if you have factory optional alloy or chrome rims, the spare MAY not match the other four wheels and tires.

If you maintain your tires properly, keep an eye on inflation pressures and replace them when they are worn, it's quite unlikely that you will ever actually NEED a spare tire in a modern automobile. I have more than 50 years of driving experience, and I can only remember two or three times when I actually needed to use the spare tire on one of my personal vehicles.

P/u trucks of 1/2 ton do have spare tires BUT the 3/4 and above with the 8 lug wheels may or may not have a spare, and anything larger than that, like a box truck, has NO spare

Yes, new ones do. And they are the same tire as the other four but just on a plain, steel wheel. Not the temporary, smaller tires that can only be driven a short distance at a reduced speed.
