The 5.4 Liter modular Ford V8 is extremely reliable if it was serviced regularly. It can be unreliable if it was neglected. They can easily run 200,000 miles with regular service.
The early 2 valve models had spark plug ejection problems, and some minor piston slap problems. The P.I. 2 valve models had the spark plug ejection issue resolved. The early 3 valve models issues with the cam phasers and problems with the OEM two piece spark plugs breaking when you tried to replace them, there's pretty specific spark plug replacement procedure that Ford specifies when replacing spark plugs on the 3 valve engines, which is meant to decrease the risk in having them break. The 4 valve 5.4L (rare) doesn't have any notable problems that I'm aware of.
